multi tool patch kit ... but lately I've been thinking I should bring a spare tube tire lever pump
I don't bring shoes. I use my cycling shoes at work. heheheheh

blinkers, (front and rear), bike tools, mp3 player (for the ride, without headphones, for safety), mobile phone, a good backpack to store your clothes for work mode.

i wear my reflectorized vest when i bike going home at night. it's a cool gift from an officemate. It makes me look like a WWF wrestler, but it works. hehe.

I bring with me tire lever,patch kit, pump, water bottle,office aparel, shoes with socks,towel, kikay kit , blinker and headlight, personal things, extra plastic bag (just in case it rain).

On bike: tail light, headlight, small locator light In Pack: extra clothes, spare tube, pump, patch kit, cable lock, minitool Alcohol, raincoat, small ziplock plastic bag for phone and wallet, plastic bag for dirty clothes. (I have clothes, shoes, a towel and toiletries in my office) On me: Helmet (of course), gloves, clear glasses (or very lightly tinted, for night riding), bandana, Mace or pepper spray, loose change. Others: Common sense to resist the urge to weave in and out of moving traffic.

A hedlight in front and a blinker at the back is a must! specially when riding b4 the sun goes up and after it goes down. Those are my very important accessories (helmet is always included) when going to work on a bike.

im bring always i go to work/ trails - headlight - blinker - water bottle - pump - tools for bike - spare tube - extra battery - mp3 player (for comfortable riding...he!he!he!) - clothes for work - extra shirt (going home) - helmet is the most important - and other accesories that is inside my back pack

inside my backpack: -extra shirt only... my kikay kit is in the office -cellphones, swiss knife, wallet, ballpen, ids... -cellphone chargers, flashdrive, hardisk enclosure with hardisk (bigat nga e), power supplies....sometimes laptop pa nga he he he.... -discman... yes discman not mp3player....(great sounding imho) -plastic bags (in case of emergency...) -tools (wrench, pacth kit, pump, etc...) -blinkers (i go home at night na eh...) -flashlight i don't bring my chain too cause i have my own parking slot at the basement and i just leave my chain there he he he...

1 - cateye EL500 2 - cat eye rear blinkers spare tube pump 1 coil lock (master lock) I leave the heavy u-bolt at parking place at work
clean clothes at the locker at work cell phone, wallet, MP3 player
Keep you eyes on the road and defensive riding techiniques

I use a tool bag to switch between bikes and inside are the following items (I find essential):
1. Chain tool (buy good ones I love the toppeak tool with a chain hanger makes derailleured chain repair easy) 2. Tire levers (zefal come in threes and are quite sturdy) 3. 6" adjustable spanner 4. multi-tool (hex-head set and drivers) 5. Flat-kit (patches and vulcanizing compound) 6. Swiss Army knife (McGyver style no rider should be without!) 7. My excess chain links (the ones you took out from your chain) take it from me guys I'm on my second chain now!
These are not necessary for me to carry but I stick them in my tool bag anyway who knows when they may come in handy. 1. Shimano Freewheel remover (for freewheel and center-lock brakes) 2. Bottom bracket remover 3. Crank puller 4. Blowtorch (a small butane powered variant I picked up at DIY shop) 5. Extra set of cables (shifter and brake; rear end always because they are longer) 7. Spare tube
These are my on-frame essentials: 1. Frame pump 2. Blinkers (front and rear) 3. Headlights 4. Coil locks (Masterlock variant)
Inside my stem mounted fanny pack: 1. Wallet (preferrably with money...but seemingly depleted lately) with essential IDs (Driver's License etc) 2. Coin purse (para sa barya at pang Max-takatak!) 3. Mobile phone (just don't text while riding!) 4. Small face towel 5. Keys
As much as I can, I try to carry everything on the bike (so I am free to move about most especially while zipping through traffic)
My suggestion, pack light, what you could leave at the office leave it there (i.e office shoes) and remember always bring along your head so the helmet can serve its purpose.

I don't really bike to work everyday... but if I plan to do it.... I'll bring my work clothes a day before.... so I dont have to carry it all along....
my usual essential - protective gears (helmet, pads, eye wear..) - allen wrench - presta converter - pump - blinkers and other Illuminants - first aid kit (I never have used it for myself but mostly for my fellow biker) - face towel - food and water - swiss knife - cellphone - money
added essentials - rain gear (but not using it even if it is raining hard... I love riding on heavy rains) - mp3 player - pocket book???
and always bring extra care and a little prayer before you go...... ride safe

yup I guess you never know what fortune may bring...actually I use the small blow torch in the workshop for anealing (heat bending) purposes, it is also powerful enough to cut through cable. It is about the size of a zippo with a torch head of course really cool gadget.
Almost forgot add to my list of essentials a rag cloth...after breaking my chain twice in a ride I realized how valueable rags are unless you want to turn your face towel into one.

since i'm in the city and quite near my place of work, here's what i bring
1) headlight and rear blinker 2) 1 set allen wrench 3) tire levers (3pcs, got it last 2003 TOF) 4) patch kit 5) cable lock 6) tire pump
then the usual gloves, mask, eyeglasses and helmet. recently
